Please be sure to pick up your child’s medication. An adult must pick up all medications. Children WILL NOT be allowed to carry medication home. Any medication left at school will be disposed of. Medication Request forms need to be updated at the beginning of each school year. A new medication form is attached and should be completed if your student will continue to need medication at school. PLEASE DO NOT WAIT UNTIL THE END OF THE SUMMER TO GET YOUR PRESCRIPTION RENEWED BY YOUR CHILD’S DOCTOR. The authorization must be completed and signed by both doctor and parent and returned the first day of school. Medication must be brought into the Health Office by an adult in a labeled bottle with the completed form. No medication can be given in the school without the proper authorization - this includes over the counter medication. Thank you and have a safe and healthy summer.

To ensure that this doesn’t happen to your child, the City of San Jose has developed an online tool to help parents find the best summer programs for their child. This tool enables parents to easily find camps, activities and events within their local area and budget. The site also has a summer learning guide with links to articles and activities to help parents support learning outside of school.
